Additional Required Forms

  • Birth certificate

  • Proof of Car Insurance (if completing Field Trip Driver Form)

Once complete, please email the forms to info@sasonline.org or drop off at the office,

We will process your admission and send the complete enrollment package within 10 days of receiving registration fee and all documentation.
